1. wrong rep range
2. eating too few calories, grams of protein, or carbs
3. bad form
4. overtraining
5. lifting too heavy or too light
6. inconsistency

I think you need to move eating too few calories to be #1, #2, and #3.

The biggest reason beginners have trouble gaining weight/msucle is because flat out they don't eat enough to do it. Sure maybe they're not lifting right for them, but more than anything it is the diet. As a beginner about anything will work from a lifting perspective.

I know, it was a joke. The biggest problem a noob has is not eating enough. I had a huge problem with that. Routine, time in the gym, form....didn't matter at all since I wasn't eating enough for it to matter.
