No insulin spike is needed. However, some resaerch suggests that it is absorbed better on an empty stomach (something to do with enzymes). That's why I take it before my workout.

actually there is research that suggests creatine is effective with out the juice/sugar it is usually taken with. I will dig this up and post it later.
