Adding weight between sets

I see a lot of suggestions to do 3 or 4 sets with reps, for example, (12,8,6,4) or (12,10,8). Is weight added between sets most of the time or not. What are some suggestions?

Yup.. as the reps get lower, up the weight.

The only efficient way to make a muscle respond by getting stronger and or bigger is with progressive overload.
