I'm new to this.  On my workout plan, I am instructed to do a "quick cardio warm-up" (treadmill, stationary bike, jump rope, etc.)  How long should this warm up be?

Also, should I do a similar cool-down (in reverse from warm up) at the end of the workout?

My workout plan is for in the gym with machines and free weights.

You can warm up with any of the methods you mention. A cool down is good after cardiac work.

I use a warm up lifting that consists of a few sets of very light weight in which ever lift I am doing. On shoulder, arm or chest day I warm up my rotator cuff as outlined here before I move on to the workout.

On leg and back days, I walk the treadmill for 5 minutes.

Don't overlook stretching before and after each movement.
