You might need to be a little more specific. What exercise are you talking about? Generally though, you should be between 70-80% your max. Also, what are your goals? Mass, toning etc...

You should start with a weight that you can easily complete the 12. This is for warm up. Increase the weight as the reps decrease.The last set should be very hard or cause failure at the 5th or 6th rep. As you are able to complete the last set, you increase the weights the next time. Make sense?
