Pre and post workout are probably the only times it may be necessary, with the pre stipulation being only if you workout shortly after you injest your meal. Other than that, whole foods fit the bill better.

What time of day is good to drink a protien shake? Also, is it more effective mixed with water, or milk??

Mix it wit water. Why take on the added calories and fat if you don't have to.

I mix mine with water also due to the same reasons Mojo stated.
