I'm doing the weight loss/tone program here. My program has me doing 3 to 4 sets with decreasing repetitions. Should I use the same weight for each set or should I increase weight as I decrease repetitions? I don't want to bulk up, just tone and develop lean muscle, if that matters.

no such thing as tone.........so take that out of your vocabulary.
To be lean you need low body fat........to shwo the "tone" whatever you wanan call it. Which is about diet.
bulk is from building muscle under fat.........
YOU WILL NOT get bulky if your diet is clean......

So say I'm doing 3 sets of 16, 12, and 10 reps. Right now for a given exercise I'm doing 90 lbs for each set, and due to muscle fatigue, i can barely complete 10 at 90 lbs for the last set. Does this mean that I should decrease the weight I use for the first set, so that as I increase weight and finish the last 10 reps, I will not be able to do anymore?

The first set is a warm up set, so start with a lighter weight... then add more weight as you make your way through the sets...
The last rep on the last couple of sets should be completable, but hard...
