Trap Strain

I've pulled my left trap 3 times in the past 2 or 3 months. Twice on a Hammer Strength Lat Machine, which I have stopped using, and once just getting up off the bench after doing skull crushers. I don't know if this is an issue of poor form, or just my predisposition to getting this injury. I'm tired of having to take time off for healing.

Also, I do stretch and warm up properly. Let me know what might be causing this and how to prevent it.

I know that this is the right answer, it's just not the one I was hoping for. It's difficult to back off when you know you can do more weight.

I'm going to the gym tonight and I'll try to back off the heavier weight for awhile to let this heal.
