Ok, I've filled in everything i was told to, I've printed off all my workouts and taken note of my recommended diet and I'm ready to begin on Monday. Any advice for how to make sure i stick at it? I know only i can do that, but any tips that might be useful?

After the pain of the first day realize it's only temporary and make sure you keep going. It'll get easier each time.

Join the military. They have effective ways of making you stick to it.

I lost 55 pounds when I joined the military.. unfortunately, in the 5 years out of the military, I put on 90 pounds... which I've lost 25 of so far.
What's driving you to make this change in your life? Whatever that one thing is, let it push you.
